Step 1: Your Choice Starts Here!

When the “Click Here to Enroll” button is clicked, a new screen will open. Fill out the information for parents or a guardian.

Students who are 18 or older may fill out their own information.

Step 2: General Information

If necessary, fill out the information for any parent or guardian outside of your home. Otherwise, leave this section blank.

Steps 3 and 4: Student Profile

The third and fourth steps are about the Student. Please fill out the information, including the birthdate of each Student and the last school attended.

If you do not know the website of the past school, fill in what you can.
